What is a disability certificate?
A Disability Certificate is a government-issued document to Persons with Disabilities (PWDs) that certifies/certifies the type and extent of the holder's disability. It is an essential document for PWDs as it entitles the holder to avail the benefits, facilities and rights of persons with disabilities. A certificate holder can access facilities dedicated to PWDs. The Government issues the certificate on the findings and recommendation of the Medical Board as to the nature of the disability.
The Government of Pakistan has set up Disability Assessment Boards across the country to help people with disabilities get their certificates. Boards usually include medical specialists, doctors, and officers from social welfare departments who take responsibility for the procedure for issuing disability certificates.
Benefits for PWDs with Disability Certificate?
If you have a disability certificate, you may be able to access financial aid, be exempted from tuition and admission fees (under certain circumstances), or be included in the recruitment process which is restricted to people with disabilities. How to Apply for Disability Certificate in Pakistan
The application process for disability certificate is as follows:

Note: If it is not possible for a PWD to physically attend the assessment, you may request your Social Welfare Office to facilitate you during the assessment process. Any exemption from being physically present for evaluation may depend on the nature of the disability and service options available in your area.

The following information about a person with disability must be provided in the disability registration form.

What if the disability certificate is lost?
If your disability certificate is lost, it can be reissued. You will have to apply to the Social Welfare Office and go through the prescribed steps again to get a new disability certificate. The documents required are the same as a copy of the First Investigation Report (FIR).

- Visit the Social Welfare Office with a copy of the FIR to collect the disability form.

محکمہ سماجی بہبود کی طرف سے معذور شخص کے حق میں معذوری کا سرٹیفکیٹ جاری کیا

بعد محکمہ سماجی بہبود کے دفتر سے حاصل کیا جا سکتا ہے۔
